Dr. Madow is a highly experienced, Harvard
trained, board certified psychiatrist. He has special
training in psychopharmacology and psychotherapy. He is
actively involved in continuing medical education and has a
wide range of understanding of legal issues as well. He has
a busy private practice and a thorough knowledge of current
treatments and controversies in this arena as well as the
ability to research issues as required.
Dr. Madow has a particular interest in
forensic psychiatry, limited to civil cases. This includes
civil competency, forensic assessment, personal injury
litigation, disability, and workplace issues. Dr. Madow
handles both plaintiff and defense cases. He has performed
numerous evaluations for competency to stand trial and an
extensive number of evaluations for the Veterans
Administration. He has also performed disability evaluations
for the state of Louisiana and will be performing disability
determinations for the state of Nevada. He has given
depositions and served as an expert witness.
FORENSIC EVALUATIONS
Forensic evaluations include a complete psychiatric history.
This consists of:
1. Chief complaint
2. History of the present illness
3. Past psychiatric history, including medications, therapy,
and hospitalizations
4. Drug and alcohol history
5. Educational history
6. Childhood history
7. Occupational history
8. Marital history
9. Work history
10. Family history
11. Religious history
12. Legal history
In addition, activities of daily living are reviewed.
A complete medical history is taken.
This includes:
1. Surgeries
2. Medical issues
3. Allergies to medications
4. Specific male or female problems depending on the
evaluee’s gender
5. Any current medications the evaluee may be taking and
from what source
An extensive mental status examination
follows.
This includes an evaluation of:
1. Evaluee’s attire
2. Grooming
3. Orientation
4. Attitude
5. Eye contact
6. Level of alertness
7. Level of anxiety
8. Concentration
9. Memory
10. Judgment
11. Mood, especially for suicidal, homicidal, and violent
tendencies
12. Affect
13. Neurovegetative signs of depression
14. Speech
15. Content of thought is evaluated, following which a
determination is made with regard to whether the evaluee is
delusional, hallucinating, or has obsessions, compulsions,
or phobias
16. Insight
17. Ability to perform abstract thought
The diagnosis is then made.
This includes:
AXIS I: Major psychiatric diagnosis
AXIS II: Personality disorders
AXIS III: Medical disorders
AXIS IV: Stressors, including medical, physical,
psychological, and social problems
AXIS V: Global assessment of function
Subsequently an opinion and extensive comment
are offered.
In addition it is necessary for Dr. Madow to
perform a complete review of the evaluee's relevant
education, military, employment, and medical records as well
as discovery and evidentiary deposition transcripts.
